About Washington Behavioral Medicine Associates, a Psychiatry and Testing Practice

Dr. Gonzalo Laje founded WBMA and still runs it as medical director. One office, on Wisconsin Avenue in Chevy Chase, Maryland. Patients come from Bethesda, Washington DC, and the nearby suburbs, some in person and some by video.

Seventeen clinicians across psychiatry, psychology, social work, and nursing, 20-plus service lines, and a research team that publishes on psychiatric genetics.

The problem

Why a 17-Clinician Psychiatry Practice Ranked for Almost Nothing It Treats

In October 2024, wbma.cc ranked for six keywords in Google’s top three and nine more on the rest of page one. Fifteen total. For a practice offering TMS, ketamine therapy, autism testing, EMDR, and psychiatric genetics, you could basically say that’s online invisibility.

Your website can list 20 services and still be invisible for 18 of them. That was the situation. A parent typing “child psychiatrist bethesda” found hospital systems and directory sites, and WBMA’s newest service lines had no pages at all.

The practice had also added clinicians and treatments faster than the site could describe them. New providers, new research papers, and a new insurance option through a partner practice all needed to show up online. Problem was, none of it did.

Helping our healthcare clients rank better in local map results.

What made it hard

Why Mental Health SEO Is Harder Than Most Healthcare Niches

Google treats mental health content as Your Money or Your Life. A thin article on bipolar disorder from a small practice loses to Mayo Clinic every time, so every page needed a clinician’s depth.

The office is a few miles from the National Institutes of Health, which shows up for the same psychiatric genetics queries the practice wants. So your local competitor is NIH, now what? You skip the authority fight and answer the exact question a parent or patient typed.

Claims language is restricted. You can’t promise a treatment outcome, imply a cure, or describe a patient result on a psychiatry site, which rules out the easiest conversion copy. Every page had to earn trust with process and credentials instead.

Then there’s the AI problem. Informational mental health searches now show an AI Overview above the results. Sitewide click-through rate on wbma.cc fell from 1.48 percent to 0.50 percent as impressions grew 18x. More people saw the practice than clicked it, and that shaped where the content plan went in year two.

The team page changed nine times in 23 months as clinicians joined and left. Stale provider bios cost you patient trust and confuse Google’s crawler, so the site needed upkeep most SEO retainers don’t include.

Every target was paired with the number it started from. That’s the only way a practice owner can hold an agency to a claim.

What we found

What Our Psychiatry SEO Audit Found on The Website

The crawl turned up a multiple-title-tags bug across templates, sitewide crawlability errors, and a Google Search Console sitemap-fetch failure. Google couldn’t reliably read the pages that did exist.

Two URLs competed for “child psychiatrist bethesda.” One week Google showed a benefits page, the next week a “complete guide for parents” post. The rankings never settled. The TMS content had the same overlap across a cluster of posts.

Impressions grew from 18,800 to 101,000 per month between November 2024 and August 2025 while clicks stayed flat. That pattern almost always means new pages are indexing and ranking on page three or four, where nobody clicks. Depth and links fix it, slowly.

Tracking didn’t exist. No GA4 conversion events, no call tracking, no Search Console history. Nobody at the practice could name the page that produced a single phone call, and that had been true for years.

The organic search & AI search domination plan

The Healthcare SEO Strategy We Built for a Psychiatry Practice

lead acquisition diagnostic

Lead Tracking

CallRail went live at onboarding in October 2024. A structured intake form with a “how did you hear about us” field and program checkboxes followed in March 2025.

Why first? A case study that says “leads went up” with no source data is a brag. “Google Organic produced 124 of 164 qualified calls” is proof. Lead tracking must always be setup before anything happens.

Website speed optimizations for WordPress websites.

Technical SEO Repairs

Fix crawlability first, because content Google can’t read doesn’t rank. Direction resolved the title-tag bug, rewrote robots.txt, repaired the sitemap error, and ran a redirect audit on the TMS cluster. Later, a plugin break took down site sections and a spam wave hit the contact forms, so the web dev work never fully stopped.

Optimizing the content for patient search intent.

Service Pages for All Treatments Offered

Ketamine therapy, bipolar disorder, cognitive disorders, Alzheimer’s evaluation, and neuromodulation each got a page of its own. Why? A patient weighing TMS against Spravato needs a page that answers that question, and the practice offering the two treatments should write it.

We also built a location page with the Google Business Profile map embedded, and a resource page linking the partner insurance practice.

Content that properly targets search intent of patients.

Mental Health Content Built Around Decision Searches

Our content plan skipped generic “what is anxiety” posts. The questions a patient asks right before booking took their place (“psychiatrist vs. therapist,” “TMS vs. Spravato,” “is ketamine therapy covered by insurance?”). Our data proves that it’s the comparison queries bring searchers with real buying intent, and AI / LLM’s pull from that format a whole lot.

The autism and child cluster came next, covering ABA, occupational therapy, speech therapy, sensory integration, and PANDAS vs. PANS. A psychoeducational evaluation guide for parents rounded it out. Forty-plus articles optimized for Google organic search as well as AI search in total, each checked for clinical accuracy.

Digital PR and Link Building naturally in healthcare.

Digital PR for a Psychiatry Practice

The content we published was quoted on 15 other sites organically. The genetic testing article picked up links some really authoritative domains, including clinicalgate.com, healthline.com, verywellmind.com, and the workplace stress guide picked up eight, including one from singlecare.com.

Dr. Laje’s bio page received placements too. AI engines cite clinicians far more often than a practice name. The bio doubles as an entity asset. We leverage it.

Different content to address different patient intents.

Topical Healthcare Content Silos

Our team mapped every article to a parent service page and built the internal links to match. Genetic testing content points to psychiatry. Autism testing points to the testing hub.

The two newest pillars, metabolic psychiatry and immunopsychiatry, launched with the links already in place. The result? The metabolic psychiatry guide went from two clicks per quarter to over 200.

Helping our healthcare clients rank better in local map results.

Local SEO, Google Business Profile Optimizations & Reputation Management

WBMA’s profile listed one primary category and skipped the services patients search for, so we went in and revamped their Google Business Profile. We updated the primary category matched to psychiatry, secondary categories for testing and TMS, a service list tied to the site’s service pages, and photos of the actual office.

The reviews were another problem. One-star reviews came from accounts with no other activity, no appointment on record, and language that didn’t match a real patient visit. We documented each one, legally reported them through Google’s review management tool, and Google removed every single one we reported that was in violation of Google’s guidelines within 2 weeks.

The Mental Health SEO Results

CallRail recorded 972 calls in all of 2025 and 1,542 calls from January 1 to September 10, 2026. Daily volume went from 2 calls to 6, a 200% increase, with Google Organic being 76% of all the calls.

But, that’s just calls. Form fills increased as well. Looking at April through August as the clean year-over-year window; Unique intake inquiries went from 85 to 126, a 48% increase. Monthly form fills from Google organic went from 9 leads per month to an average of 14.

The Mental Health SEO Results After 16 Months

Organic sessions went from 482 to 3,037 per month, up in 13 of 15 months. Clicks from Google went from 369 to 2,241. Impressions went from 24,974 to 452,534, an 18x increase. Average position moved from 38.9 to 10.0, with a best month of 6.0 in March 2026, the same month the crawl fixes and silo architecture closed. Keywords in Google’s top three went from six to 203, positions four through ten went from nine keywords to 282, and 92 percent of all clicks now land on page one. Domain Rating rose from 26 to 33. Total site sessions grew from 1,133 to 6,130 per month, and direct traffic tripled, which is what happens when a patient sees a practice in Google, closes the tab, and types the name in a week later.

AI search results

Thirty-two pages on wbma.cc now appear as cited sources across Google AI Overviews, Google AI Mode, Perplexity, Gemini, and ChatGPT (Ahrefs, September 2026 snapshot). CallRail tagged 14 calls to AI search engines in 2026, and six intake forms arrived with ChatGPT tracking parameters.

Why This Psychiatry SEO Strategy Worked

The site couldn’t rank for services it didn’t describe, so year one built the pages and the articles. Impressions rose for five months before clicks did, and that stretch is where most practices fire the agency. Dr. Laje’s team trusted the process and stuck with it. And won big for that.

Specialty pages beat generic ones for patient inquiries. A parent searching “eTNS ADHD” or “TMS Chevy Chase” is a buyer the referral network wasn’t sending. Ninety-six percent of eTNS inquiries and 86 percent of TMS inquiries named Google.

Psychiatry inquiries still came through referrals, and that’s fine, since search filled the service lines referrals couldn’t reach.

Structure came before links. The silo map from February 2025 and the crawl fixes from March 2026 pushed authority to the pages that mattered. The best month on record landed the same month those fixes closed.

Find Out Which of Your Service Lines Google Can't See

You probably offer treatments your website never mentions, or mentions on a page stuck on page four. A healthcare SEO audit from Direction shows you which service lines have no page and which pages compete with each other. The same audit shows where AI engines cite your competitors and skip you.

Mental Health SEO Questions Practice Owners Ask Before Hiring

Call tracking, set up before the campaign starts. CallRail assigns a tracking number to organic search visitors, so the source gets tagged the moment a Google Organic visitor calls.

WBMA’s form asks “how did you hear about us,” and that data is useful. A checkbox can still hide a Maps click or a paid ad. The 76 percent figure on this page comes from the call tracking, with the form data reported separately.

Six months, in this case. WBMA’s impressions grew for some time while clicks stayed flat, when average position jumped from 36 to 11.6 in two months.

A practice with an existing content base moves faster. A practice starting from 15 page-one keywords should plan for two to three quarters before the traffic curve bends.

Every article gets a clinical accuracy review before publishing, and nothing promises an outcome. The copy describes what a treatment is, who it’s for, what the research says (with named sources), and what to expect at the visit.

You’ll find the same approach in Direction’s HIPAA marketing guidelines. Slower than a content mill, and the reason the pages rank against hospital systems.
